Procedure for Becoming ESI PG Member

Dear Applicants,

Please find the detailed step-wise procedure of becoming a PG member:

1. Step 1: Please Click Here to download the HOD certificate: HOD Certificate Form
2. Step 2: Fill the particulars and get the form signed by your HoD and save it as a PDF document on your computer.
3. Step 3: Find the ESI number of your proposer and submit the application form online with all the details and attachments.
4. Step 4: After submission, ask your proposer to log in to the website as a member with their credentials and forward the application. (Please note that the Proposer will not receive any mail from the ESI and they have to log in with their credentials and do the needful).
5. Step 5: The application moves to the Secretariat only after the proposer accepts the application.
6. Step 6: You will get an email with the membership details after the Secretariat approves the application. Please keep checking the spam mail also.

• A person enrolled for training in the Endocrinology from a recognized teaching / academic department in India towards the DM / DrNB degree course shall be eligible to become a PG member for a duration of 3 years from the date of enrolment or completion of the course, whichever is earlier.
